In 1997, Westminster Towers became the first retirement campus in South Carolina to be accredited by the National Continuing Care Accreditation Commission. Our mission is to provide services that inspire, encourage, and empower seniors while demonstrating the love of Christ to support individual well-being.

Today, our community offers 131 Independent Living apartment homes, 29 Assisted Living suites, 22 Medicare Certified Short Term Rehabilitation rooms, and 22 Long Term Care rooms. In 2008, HomeBridge, an extension of the ministry of Westminster Towers was founded, providing non-medical caregiver services to Westminster Towers residents and the community.

DEPARTMENT: HEALTH CARE AND REHABILITATION CENTER

Westminster Health and Rehabilitation is a Not-for-profit organization that provides Skilled Nursing and Rehabilitation services. We serve our residents with Integrity, Respect, Excellence, while striving to Enrich their lives.

We are searching for individuals who will provide quality, compassionate care to the seniors we serve. The successful candidate will work well within the interdisciplinary team, and will help to create a comfortable and positive environment for our seniors and their families.

Tasks:

  • Provide guidance to LPNs and CNAs
  • Administer medications, provide vaccinations, and render treatment to senior adults.
  • Communicate with physicians, CNAs, and other nurses regarding resident care.
  • Participate in creating a plan of care and charting for each resident.
  • Respond to inquiries and concerns from residents and their families.
  • Assist with patient education for discharge planning.

Skills and Requirements:

  • 3+ years RN experience, preferably working with seniors.
  • Valid Nursing License.
  • Willingness to work a flexible schedule and fill in when needed.
  • Working knowledge of South Carolina health care laws and regulations.
  • Current CPR Certification.

Work Schedule:

  • 3rd Shift and Weekends — All shifts available.

Clinical Unit Coordinator

REPORTS TO: Director of Health Services

DEPARTMENT: Health Care and Rehabilitation center

FLSA: Non-Exempt

Primary Function

This position is responsible for assisting the Nursing Department and the Director of Health Services in ensuring regulatory Compliance for the Health and Rehab Center. Responsible for leading the Nursing Department in the absence of the Director of Health Services. This position is responsible for putting new admissions and readmissions into the EMR system.

Dimensions

Number of employees that position directly supervises: 2-6 Certified Nursing Assistants/Nursing Assistants

Essential Functions

  1. May be required on occasion to work on unit as needed to meet regulatory staffing compliance.
  2. Responsible for supporting Charge Nurses with the clinical coordination for admissions on each unit.
  3. Participate in on-call nursing rotation, as requested.
  4. Monitor for accuracy and completion of resident monthly and weekly weights. Responsible for addressing any issues of non-compliance and ensure proper follow through with nursing recommendations.
  5. Assists DHS in monitoring and improving existing systems for data collection relating to resident care for accuracy and appropriate follow through i.e. bath schedule, body audits, MARs and TARs.
  6. Assist Charge Nurses in supervising day to day tasks of nursing department. Communicate concerns to DHS.
  7. Assist DHS with gathering information for QA Compliance Reports for Quality Indicators.
  8. Supervise and monitor the completion of documentation and address and report any compliance issues to DHS and Administrator.
  9. Assists with wound care program to include proper weekly measurements and weekly reporting to DHS as needed.
  10. Assists DHS with training/ in-services to staff.
  11. Direct implementation of Resident’s Rights at all times.
  12. Demonstrate and monitor all facility safety policies and procedures for compliance.
  13. Must maintain and monitor for compliance of all privacy rules and regulations pertaining to Resident Rights and Health Insurance Probability and Accountability Act (HIPAA).
  14. Must complete an annual mandatory HIPAA training session.
  15. Works closely with therapy to initiate and monitor therapy orders as needed.
  16. Report concerns to the DHS.
  17. Support Charge Nurses in conducting daily resident rounds and assist to initiate action plans as necessary.
  18. Monitor resident care plans to ensure up-to-date as needed.
  19. Must have flexible work availability schedule, including days, evenings, nights, weekends, holidays.
  20. Perform other duties as assigned.

Critical Knowledge, Skills and Expertise

  1. Current South Carolina Nursing License
  2. Minimum 3 year nursing experience in a long-term skilled nursing facility preferred.

Physical Demands and Working Conditions

  1. Heavy physical demands.
  2. Job requires the manipulation of the physical mass of a resident on a daily basis.
  3. Exposure to body fluids.
  4. Exposure to potentially infectious materials.
  5. Rotation of work hours is to be expected.
